Prescribe Like You Normally Do
Use your standard clinical workflow. Submit prescriptions through Surescripts, our secure portal, or eFax.

We Compound and Deliver
Each prescription is compounded individually according to your written instructions and shipped directly to eligible patients in Colorado.

Monitoring and Support
Our pharmacy team is available for prescription clarification and coordination needs.
All clinical follow-up remains with the prescribing clinician.

How To Prescribe
Kazmira Pharmacy accepts prescriptions through multiple secure channels. All prescriptions must include concentration, quantity, directions for use, and refills if authorized.

eRx
Submit prescriptions through Surescripts (LifeFile). Search for: Kazmira Pharmacy
Ensure your eRx includes the patient’s name, date of birth, formulation strength, quantity, and instructions for use.

eFax
Send written prescriptions to our secure eFax line (720-334-8998)
Include clinician NPI, signature, date, formulation strength, quantity, and clear directions for use.

Portal
Upload prescriptions through our secure pharmacy portal (available upon request)
Attach a complete prescription with all standard elements required for compounding, including any authorized refills.

Prescriptions must include formulation strength, quantity, directions for use, and any authorized refills. Clinician NPI, signature, and date are required. The pharmacy compounds the medication exactly as written.
We utilize a network of ISO 17025 certified third party analytical labs. Documentation is available for provider records or patient reassurance.
No. All clinical decisions, including appropriateness, formulation strength, and dosing, are determined solely by the licensed prescriber. The pharmacy compounds only according to the written prescription. Partner and patient educational tools are available in the Provider Onboarding Kit.
Most prescriptions are compounded and shipped within 2 business days once all required information is received. Patients will receive shipping and tracking updates email and can contact pharmacy directly with fulfillment questions.
